“Please bring your child”

Your child MUST be 5 on or before August 1, 2024 to enroll in kindergarten

The child MUST have a COPY of:

-Social security card

-Physical examination – (Kindergarten students are required to have a physical examination from a health care provider before entering school))

-Immunization record 

Parent/guardians are also required to provide ONE of the following documents to indicate the child’s age:

-Birth Certificate (state issued)

-A statement by a local registrar or a county record certifying the child’s date of birth

-An attested baptismal certificate

-A passport

-An affidavit of the date and place of birth by the child’s parent/guardian

-Previous school records

-Military identification records

IF THE SCHOOL DOES NOT RECEIVE THESE BY SEPTEMBER 11, 2024 YOUR CHILD WILL NOT BE ALLOWED TO STAY IN SCHOOL
